There are quite a few questions, but it will help us learn more about you to determine if we are a good fit for you. It should take about 15 minutes to complete in its entirety. Once we have the completed questionnaire, I would be happy to schedule a video chat with you to go into the surrogacy process more in depth. I am also open for any questions you may have in the meantime!
